Clients have the right of Self Determination and are Empowered as Partners in:
assessing their care/service needs;
planning and developing goals and time frames for their outcomes;
implementing and achieving those goals; and
evaluating and revising their care plan.
Clients receive care in a Safe Environment that is:
supportive of choices, including the right to refuse service;
respectful of confidentiality of person and records.
Clients are Respected and Valued as Individuals by:
accommodating the provision of care to the social/cultural customs, values and beliefs of the client;
acknowledging preference for caregiver;
practicing respect for privacy and dignity;
providing care in a courteous compassionate manner; and
supporting and respecting decision making, and selfcare and wellness roles.
Clients receive Efficient, Effective, Outcome-Based Care, Delivered through:
a team of compassionate, knowledgeable staff;
programs and services that are current research/best practice based;
a service schedule that provides continuity of care/service and caregiver;
a "holisitic" client focus i.e. inclusive of family, and significant others;
care plans that are individually designed to meet and exceed outcomes and timeframes;
ongoing assessment of client status and care needs; and
ongoing monitoring and assessment of the quality of care delivered and review of client satisfaction.
Client's Right to be Informed is promoted by:
providing appropriate health information while respecting privacy;
assuring full access to their care plan;
assuring full knowledge of their business contract;
informing them regarding ParaMed operations;
informing them in writing of the process to request a review of services;
investigating their request for review of services, fully and without prejudice; and facilitating access to other community resources, as client advocate.
